Obtaining Your Medical Record

If you need your medical records, we will be happy to send them to you. A patient and/or their legal representative may inspect and/or obtain a copy of their personal health care information. They may also have copies sent to a third-party provider.

Medical Record Release of Information Form

RMH requires a completed and signed authorization form to release health information to anyone, including the patient. You can obtain a free copy of the authorization form here.

Q. How do I request a copy of my medical records?

Please complete the release of information form. Mail or fax completed, signed request form directly to the Medical Record Department listed above. You may also drop off the request at Roundup Memorial during normal business hours.

Q. How long will it take to receive my records after I submit my request?

Once the Medical Record Department receives your authorization, it may take up to 30 business days to process. Occasionally delays occur for active records that are needed for patient care or for inactive records that are in storage. We will attempt to contact you if extra time is needed to process your request.

Q. How will the records be delivered to me?

We will deliver the records in the method that you choose. If the records are too large, they will be mailed to you. If you choose to have your records emailed to you, please note that Health information sent via unencrypted email may place risk of inappropriate access to the information contained within the email. The patient will accept this risk if they direct Roundup Memorial to send their health information this way.

Q. Is there a cost to obtain my records?

Patients can have their records and x-ray images sent to another healthcare provider for free. Obtaining a copy of your medical record for your personal records may include a fee of $15 for retrieving and processing your records.

If a patient would like to obtain a CD of their x-ray images there will be a $5.00 charge.
