Q. How long will it take to receive my records after I submit my request?

Once the Medical Record Department receives your authorization, it may take up to 30 business days to process. The patient will accept this risk if they direct Roundup Memorial to send their health information this way.

Q. Is there a cost to obtain my records?

Patients can have their records and x-ray images sent to another healthcare provider for free. Obtaining a copy of your medical record for your personal records may include a fee of $15 for retrieving and processing your records.

If a patient would like to obtain a CD of their x-ray images there will be a $5.00 charge.
